The process for these Adoption Contracts is that the person who initiates these forms will fill in the information related to the Dane being adopted. The next signatures are for the potential adopter(s). They should be instructed to review these documents prior to going to meet the Dane so they have time to carefully review what they are being asked to sign. After reviewing the document they can click on the upper right corner under More Options and Click Finish Later. Then when they arrive at the fosters home if they decide to move forward with the adoption they should go back to the original email from DocuSign and click on “Review Document” to finish the signing process.
Chapter Coordinators are encouraged to log into DocuSign and initial these documents from within their DocuSign account. This will then provide visibility into the status of the documents are well as an easy way to view the final documents.
Please coach the adopter as to how to work with these documents to ensure a smooth process.

- Adoption Contract Individual – To be used for the adoptive individual with no training or spay neuter addendums (01/25)
- Adoption Contract Individual – with Training Addendum – To be used for the adoptive individual where both a Training addendum is required (01/25)
- Adoption Contract Individual – with Spay Neuter & Training Addendums – To be used for the adoptive individual where both a Spay Neuter & training addendum are required (01/25)
- Adoption Contract Individual – with Spay Neuter NO Training Addendums – To be used for the adoptive individual where both a Spay Neuter & NO training addendum required (03/25)
Family Contracts
- Adoption Contract Family – To be used for the adoptive family with no training or spay neuter addendums (01/25)

A link will be included in the email with the proper link based on the Waiver Form to Sign selection. You will receive a copy of the email that is sent. Keep this email in case the person who you are asking to sign cannot locate the email. You would then be able to forward them a copy of the email.
